Diabetes mellitus (DM) is primarily characterized by high blood glucose levels (hyperglycemia), polydipsia, and polyphagia. Vitamin C has the antioxidant capacity and β-carotene has been exhibited to reduce oxidative LDL and prevent atherosclerosis [16,22]. Physical activity has always been prioritized by physician’s advice for DM’s management and many studies have suggested them effective in not only diabetic management but also in reducing the risk of age-onset diabetes alongside other age-onset diseases including cardiovascular diseases [92,93,94]. Sedentary lifestyle is a critical risk factor of DM, especially via insulin resistance and evidence suggests that a healthy exercise routine reduces insulin resiWhile ex-vivo somatic gene therapy generates efficient β-cells, thus treating insulin deficiency [129,144], in-vivo somatic gene therapy prioritizes lowering blood glucose via generating both insulin [145,146] and noninsulin genes [147,148,149] (Gck, PTG etc.). While sharing a somewhat common strategy with metformin therapy, most importantly it does not share the risk factors of metformin therapy [150]. Somatic gene therapy shares a common disadvantage with stem cell technology as both these technologies are in the development phase and thus unavailable for general clinical practice. However, it is expected that DM would become globally extinct while these technologies become available for the general public. This review focused on the epidemiology of diabetes, the complications of diabetes, and the appropriate management of the disease.Diabetes mellitus has become a serious and chronic metabolic disorder that results from a complex interaction of genetic and environmental factors, principally characterized by hyperglycemia, polyuria, and polyphagia. Uncontrolled high blood sugar can result in a host of diabetic complications.DM is one of the most common metabolic disorders that is increasing at an alarming rate all over the world [1,2,3]. The number of patients with DM has quadrupled (from 108 million in 1980 to 422 million in 2014) within 34 years only, while the worldwide incidence of diabetes among adults over 18 years of age has risen to 8.5% (2014) from 4.7% (1980) [1]. The WHO estimates that diabetes will be the 7th primary cause of fatality by 2030 [2]. There are mainly four common types of DM.

Evidence-based guidelines for the treatment of type 2 diabetes mellitus focus on three areas: intensive lifestyle intervention that includes at least 150 minutes per week of physical activity, weight loss with an initial goal of 7 percent of baseline weight, and a low-fat, reduced-calorie diet; aggressive management of cardiovascular risk factors (i.e., hypertension, dyslipidemia, and microalbuminuria) with the use of aspirin, statins, and angiotensin-converting enzyme inhibitors; and normalization of blood glucose levels (hemoglobin A1C level less than 7 percent).Multifactorial interventions to manage cardiovascular disease risk factors (i.e., blood pressure, cholesterol, microalbuminuria) in patients with type 2 diabetes have been shown in well-designed clinical trials to decrease mortality rates.10 Daily low-dose aspirin is recommended for patients with type 2 diabetes and coronary artery disease (CAD), those older than 40 years, and those who have additional risk factors for cardio-vascular disease (e.g., family history of cardiovascular disease, hypertension, smoking, dyslipidemia, albuminuria).7 Statins are recommended for patients with type 2 diabetes and CAD, and for patients with diabetes without CAD who are older than 40 years and have one other cardiovascular disease risk factor.7 Angiotensin-converting enzyme inhibitors and angiotensin receptor blockers are mainstays of treatment for patients with micro- or macroalbuminuria.7Medications target one or more of these defects (Table 2).11–13 Average absolute reductions in A1C for each class of medication range from 0.5 to 1.0 percent for exenatide (Byetta), pramlintide (Symlin), and alpha-glucosidase inhibitors to 1 to 2.5 percent for sulfonylureas and metformin.14 Reviews have reported that mono-therapy with any oral hypoglycemic agent is superior to dietary management or placebo in reducing A1C values, but the studies are so heterogeneous that the expected A1C reduction attributed to any class of medication should be interpreted with caution.15,16 For example, sixPramlintide is an amylin analogue indicated for use in patients with type 1 diabetes; it is rarely used to manage type 2 diabetes.11 When pramlintide is initiated, the insulin dosage should be reduced by 50 percent to avoid potentially severe hypoglycemia.
